Stronger Roots operates with a small, hands-on team. You will have your own caseload and responsibilities, but you must also be willing to notice gaps, help with an intake, follow up on an unresolved issue, support a teammate, and step in when the program reasonably needs help.
We need someone who takes ownership, follows through, communicates, solves problems, and does not need to be told every next step.
About the Position
Stronger Roots is seeking a California Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) for our Recuperative Care program.
This is not a traditional nursing position. It is a clinical case-management role combining behavioral-health support, client engagement, care coordination, documentation, and clinical follow-up.
Our clients are often more behavioral-health focused than medically intensive, with mental health, substance-use, and co-occurring conditions. Behavioral-health experience is strongly preferred, although we are willing to train the right candidate.
The position manages approximately 35–40 clients and works closely with our RN, case-management team, healthcare providers, behavioral-health providers, and community partners.
Key Responsibilities
- Manage a caseload of approximately 35–40 clients.
- Complete and assist with intakes, assessments, care plans, and discharge planning.
- Maintain regular client contact and identify medical, behavioral-health, substance-use, and social-service needs.
- Complete daily progress notes and required case-management documentation.
- Review medical records, medications, discharge instructions, behavioral-health information, and provider recommendations.
- Provide clinical support within the LVN/LPT scope of practice and appropriately escalate concerns.
- Coordinate medical care, behavioral health, psychiatry, substance-use treatment, pharmacy, Home Health, specialty services, and other community resources.
- Coordinate appointments, referrals, transportation, prescriptions, and provider follow-up.
- Communicate with hospitals, Managed Care Plans, healthcare providers, behavioral-health providers, and community partners.
- Participate in case conferences and interdisciplinary team meetings.
- Help clients address barriers involving treatment, medications, benefits, housing, transportation, and recovery.
- Maintain accurate and timely electronic records.
Preferred Experience
Behavioral-health experience is strongly preferred, particularly working with individuals with:
- Mental health conditions and serious mental illness.
- Substance-use disorders.
- Co-occurring mental health and substance-use disorders.
- Trauma and behavioral dysregulation.
- Medication-adherence challenges.
- Homelessness and housing instability.
- Difficulty consistently engaging in treatment.
- Experience in psychiatric services, behavioral health, substance-use treatment, crisis services, community mental health, Recuperative Care, homelessness services, or case management is a plus.
Qualifications
- Current California LVN or LPT license.
- Strong communication, organization, and follow-through.
- Comfortable managing approximately 35–40 clients.
- Strong professional boundaries and client-engagement skills.
- Ability to recognize and appropriately escalate clinical or behavioral-health concerns.
- Strong computer and Electronic Health Record (EHR) skills.
- Ability to complete accurate and timely progress notes and electronic documentation.
- Comfortable working independently while remaining part of a small interdisciplinary team.
- Experience with trauma-informed care, motivational interviewing, de-escalation, crisis intervention, CPR/First Aid, Narcan, Medi-Cal/CalAIM, or ECM is preferred.

This is a full-time, in-person, boots-on-the-ground position. It is not a work-from-home position and is not intended for someone looking for a second job or simply additional nursing hours.
