Enhanced
Care Management (ECM)
Navigating the healthcare system can be overwhelming. Our ECM services provide a dedicated Lead Care Manager to help coordinate all aspects of your health and social needs — a true “whole-person” approach that goes beyond medical care alone.
- Comprehensive assessment and individualized care planning
- Coordination of primary care, behavioral health, and dental care
- Health promotion and medication management support
- Transitioning care from hospital or facility to community settings

Community Supports (CS)
A stable home is the foundation of recovery and good health. Through CalAIM, we assist Medi-Cal members in finding, securing, and maintaining safe housing.
Additional Support Services
We collaborate with a wide network of partners to ensure individuals and families have access to the resources, guidance, and support they need.
Probation & Parole
We partner with probation and parole departments to provide stable housing and wraparound support for justice-involved individuals navigating reintegration.
County Mental Health
We coordinate with county behavioral health programs to ensure individuals with mental health needs receive integrated housing and care support.
Homeless Shelters
Partnering with local shelters allows us to connect individuals experiencing homelessness with stable, structured housing and ongoing support.
Jails & Institutions
We collaborate with correctional facilities to pre-arrange housing and services before release — reducing recidivism and promoting lasting stability.
Family Courts
Our team provides housing stability documentation for individuals navigating family court proceedings, supporting family reunification efforts.
Hospitals & Emergency Departments
We partner closely with hospitals and emergency departments to provide immediate post-discharge housing placement and wraparound care for individuals who have no stable housing to return to.
Do You Qualify?
CalAIM services are covered at no cost for eligible Medi-Cal members.
If you are enrolled in a Medi-Cal managed care plan in Solano or Napa County and face housing instability, a recent hospital discharge, or complex health needs — you likely qualify.
